Series Description: The Urine Sample Request Records consist of computer-generated reports identifying employees who were due to submit routine urine samples. Information in the records includes employee names and identification numbers, dates on which the samples were due, schedule codes indicating the frequency of sampling for each employee, analysis types, and dates of previous sampling.
