Series Description: This series consist of photocopies of two forms, titled Report of Work Absence and Treatment Record, compiled monthly by the Hanford Environmental Health Foundation for the Health Surveillance System and used to monitor employee illnesses and accidents. Personal data on the forms include names, social security numbers, job titles, supervisor names, and work locations. Information regarding medical treatment includes the health event, subsequent treatment, medical diagnosis, and reason for medical visit. The Report of Work Absence form indicates dates of absence and notes employee hospitalization and malignant medical conditions, if applicable.
