Series Description: This series contains a Type A Occurrence Report that documents the death of a Savannah River Plant employee. Information includes employee name and identification number, social security number, date and place of birth, address, sex, race, highest level of education, marital status, spouse name, dates of hire and termination, reason for termination, type of employee, job title or occupation, previous work history, work location, date and cause of death, place of death, payment of death benefit and date, and last known vital status.
