System Description: This system tracks occupational injuries and illnesses incurred by operations and construction employees at the Savannah River Site (SRS). Data entered into this database is collected from the SRS Injury/Illness Report from the Medical Department. Outputs from the system include Individual Accident/Incident Report Department of Energy Form 5484.X, and South Carolina Workers Compensation Employer's First Report of Injury or Illness Form 12A. Data includes employee name, social security number, age, home address, organization and division names, supervisor name, site location of injury, date and time of occurrence, affected body regions, workdays lost and restricted, and cause of injury.
