Series Description: This series consists of completed forms used to estimate employee exposure when dosimeter badges were lost, misplaced, damaged, or not returned. Information includes employee name; payroll number; location and department; status of badge as lost, missing, damaged, or unreturned; and an estimation of exposure. Information is also provided on the distribution of nonroutine badges, including employee name, payroll and badge number, date, dosimeter type, work area, cycle issued, background and dose readings, and comments.
