Series Description: This series consists of Qualification Card Verification forms which were completed each time a thermoluminescent dosimeter (TLD) was provided to an employee or visitor at the Savannah River Site. Each sheet provides the name, social security number, office, building, room, and telephone number of the person who received the TLD; indicates whether "RCA" and F-Wing Access were required and whether the person getting the TLD received radiation training; and provides the dates the recipient last had a whole body count and respirator fit test. The date and recipient's signature are also included.
