Series Description: This series contains forms, logsheets, and data sheets containing raw dosimetry and bioassay test results for data entry. Employee information includes name, badge number, and work location. Dosimetry data includes job number, date, field name, pull day, badge number, health physics area, and cycle date. Memoranda address discarded, lost, or contaminated badges. Logsheets provide name, code, date, and open window and shield dosimeter readings. Bioassay information consists of badge number; work area; date and time of sample; routine counts per minute; special counts per minute; volume of sample in milliliters; bioassay test results for tritium, uranium, and plutonium-238 and -239; and required reruns of sample and bioanalysis.
