Series Description: This series consists of computer-generated reports of special test badge results with Special Dosimetry Requests attached. Information includes name; social security or payroll number; department; area; cycle number; date and time of exposure; length of exposure; results for shallow, deep, E1 through E4, extremities, and thermoluminescent neutron dosimeters; dose rate given for beta-gamma and gamma; and reason for request. Measurements are given for whole body and extremity exposures.
