Series Description: This series consists of reports of onsite deficiencies, which appear to be small incidents or infractions of safety rules in radiological areas. The Radiological Deficiency Reports provide building locations, descriptions of events (e.g., spills, elevated radiation levels, etc.), descriptions of the safety deficiencies which caused the events, immediate corrective actions taken, and preventive actions subsequently adopted. Employee external exposure rates are also provided in a small number of the reports.
