Series Description: This series consists of medical case files which Occupational Health maintains for each employee (including contract employees) at Rocky Flats. Included are initial health history forms, pre-placement drug screening test results, Employee Medication Limitation Reports, Biochemistry Profiles, eye examination results, cholesterol testing results, injury and accident reports, electrocardiogram reports, spirometry [lung capacity] reports, audiometry test results, treatment records, and termination forms. Data include employee names, identification numbers, addresses, hire dates, termination dates (if any), department assignments, work phone numbers, salary rates, and supervisors' names. Health records of female employees also include forms explaining the hazards of radiation exposure during pregnancy.
