Series Description: This series consists mostly of records used to monitor the status of the Beryllium Surveillance Program. Included are Beryllium Response Health Surveillance Participation Forms, which are sent to current and former employees to inform them of the program and solicit their participation. Data on the forms include employees' names; addresses; and social security, identification, and telephone numbers. The series contains appointment schedules for program participants, which provide employee names, addresses, and medical examination dates. Records in Room 172 also include a file indicating which employees tested positive or negative for beryllium.
