Included are employee termination notices, personnel action requests, employee rating reports, medical termination forms, recruitment records, employment applications, college transcripts, Personnel Security Questionnaires, badge requests, and personnel performance reviews. These records include the following information: employee names and numbers, social security numbers, maiden and other names, addresses, telephone numbers, spouses' names and addresses, birthdates, places of birth, sex, race, educational backgrounds, marital status, hire and termination dates, reasons for terminations, job titles, work histories, and work locations.
