Series Description: This series consists of completed employee questionnaires used as source records for the Occupational Health History Master File. Information pertaining to family, educational, occupational, and military service histories are contained in the records. Personal identifiers include names, payroll numbers, social security numbers, prior names, sex, race, birthplaces, and birthdates. Parent, children, and sibling names, addresses, and relevant mortality data (e.g., death place, age of death) are also contained within the records. Occupational histories provide prior work experience (e.g., occupation titles and descriptions, dates, employers, and work locations) as well as Hanford site information (e.g., job titles and descriptions, hire and termination dates, organizational codes, locations by building and area, and contractor company). Container Numbers 35006-35008 contain questionnaires with similar information completed by employees of Du Pont Chemical Company, a Hanford contractor. The dates covered in the records are 1940 through 1977. Container Number 126009 contains a notebook, labeled Case Control Selection, which provides the methodology used to select controls for tracking information.
