Series Description: This series consists of mortality record cards which contain health history data on deceased Hanford employees. The data include names, ages, sex, payroll numbers, social security numbers, job titles and tenure dates, dates and places of birth, places and causes of death, medical histories (i.e., X-rays, family history, smoking history, blood pressure, height, and weight), and autopsy diagnoses. Sections are included for bioassay and occupational radiation exposure results, but no measurements are given. Also included are notebooks containing logs of death certificates for construction and operations personnel which were not entered onto record cards. The notebooks are dated 1981; the mortality record cards are undated.
