Series Description: This series documents the creation of employee questionnaires used in a Department of Energy headquarters directed program. The program's goal was to test an occupational coding and job task analysis system. Hanford Environmental Health Foundation collected the data and sent them to DOE for analyses. These records contain drafts of questionnaires; a copy of the standard cover letter sent to employees; and a list with the names, addresses, social security numbers, job titles and phone numbers of employees contacted. Analysis revealed that the data were insufficient, and the system was never implemented.
