Series Description: This series consists of carbon copies of Waste Processing Request Forms. The forms provide waste characteristics (liquid or solid, acid or neutral) and indicate when and from which buildings or departments the processing requests originated. Also noted are whether the waste should be shipped offsite, transferred to another location onsite, or disposed of (presumably onsite). Signatures denote approval by Health Science, Waste Processing, and Radiation Monitoring.
