Series Description: This series consists of requisition forms used to document information about nuclear material samples sent to the Analytical Laboratory for testing. The forms track the materials as they were transferred to the laboratory from other site areas and include the location where the samples were analyzed and the types of tests requested for each sample. Each form notes a date, to and from accountability areas, operator number, cost account number, name and location of person making the request, name of the person to whom the sample request was sent, sample type, cast number, vial number, receiver weight, measurement unit of the test to be performed (usually in parts per million), and laboratory remarks.
