Series Description: The Occurrence Reporting and Processing System (ORPS) is used to document radiological incidents and occurrences at Rocky Flats and other Department of Energy (DOE) sites. Facility managers create and modify reports of radiological occurrences on workstation personal computers (PCs), as required by DOE Order 5000.3A. The reports are then downloaded to the ORPS database, after which they are available for review by other site personnel through computer terminals or PCs having the necessary capability. The database can be used to generate various reports in hard copy, including Notification Reports, which must be submitted to DOE within 24 hours of occurrences, 10-day followup reports, and final reports. All of these reports provide building numbers and managers' names; job positions (but not names) of employees involved; occurrence report numbers, dates, and descriptions; and explanations of immediate actions taken. The 10-day followup and final reports also list probable causes, describe corrective measures needed and subsequently taken, and evaluate effects on the environment and/or employee health. Information in the ORPS database can be used to identify and analyze trends in occurrences and to develop preventive measures. Records do not include personal identifiers.
