Series Description: This series consists of copies of Atomic Energy Commission standard forms titled Monthly Summary - Accident, Occupational Disease, and Fire Experience prepared either by or for two plant food service providers, Szabo Food Service and ARA Services. Categories of information on the forms include reporting organization or office; contract number; date form was completed; location; area or operations office; type of operation; and information on personal injury and vehicular accidents, fires, and property damage. While most of the forms do not report accidents or incidents, one form pertained to an alleged food poisoning incident.
