Series Description: This series contains employment and exposure data for individual workers. Included are cover sheets indicating employee names, hire and termination dates, and position titles and effective dates. Attached to each cover sheet is a coded sheet of information concerning the employee, including identification number, name, sex, education code, hire and termination dates, birthdate and place, tracing information, death date, death certificate number and information, autopsy data, health physics internal and external exposure information, date of first positive urine sample, urinalysis results, and date of last known vital status. The cover and coded information sheets are accompanied by copies of the employee's death certificate, urinalysis report, radiation exposure summary, health science data system report (which is a complete health summary and urinalysis detail), Health Physics TLD [thermoluminescent dosimeter] Badge Annual Dose Report, work location information, and radiation dosimetry body count results.
