Series Description: This series consists of forms which document the incidence of breast cancer among female workers at Hanford. Forms list the study identification number, case/control match-unit number, case/control status, Health Surveillance System diagnosis code, diagnosis code modifier, and reference date. Employee identifiers in the records are name, social security number, birthdate, and race. Date of diagnosis; tumor location; pre-employment examination date; and obstetrical, gynecological, comprehensive medical, and occupational histories are also provided for study participants. In some cases, occupational radiation exposure records are included.
