Series Description: This series consists of several reports, all of which appear to trace employee health events which required medical care. Titles include Workmen's Compensation Claims Prepared, OSHA Recordable Cases, Ambulance Reports, Job Incurred Treatments by Contractor, and Total Treatment Visits by Service by Contractor. The reports provide a variety of information, including social security numbers and names; medical facility visit dates; and codes indicating diagnosis, "operation," "action," and medical status. Additionally, the series contains a list of contractors with monthly totals for physical examinations, medical treatments, preventive care provided, and surgeries performed.
