Series Description: This series consists of photocopies of death certificates for former Hanford employees. Attached to some certificates are forms titled Health and Mortality Data Input Information. Data on the certificates and corresponding forms include birthdates, death dates, sex, race, cohort (i.e., construction or operations personnel designation), and an entry from the "Nosology ICD-9 Submission Code " (i.e., cause of death in standardized code from the International Classification of Diseases, Ninth Revision).
