Series Description: This series consists mostly of computer-generated reports used for corrections to the Health Surveillance System (HSS) database. Types of corrected data include employee identification numbers, sex, birthdates, types of disease, treatments, and HSS codes. The series also contains reports with cumulative data of employees sorted by International Classification of Diseases codes and diagnoses; forms used to edit data which had been entered incorrectly due to errors in the input source documents; and a list of medical histories, physical examination forms, and other documentation which were not integrated into the HSS.
