Series Description: This series consists of forms used to correct, add, or delete data in the Mortality Study database. The forms were prepared by the Hanford Environmental Health Foundation and then incorporated by Pacific Northwest Laboratory into the HMO.DBF file. Some of the forms contain nosology [disease classification] corrections and have the corresponding death certificates attached. One folder, titled Hanford Malignancy File, contains a computer-generated report of malignancy data. Another report lists corrected employee data, including names, addresses, and social security numbers.
