Information provides employee name, payroll number, job title, work location and building, and supervisor's name; date, time, and cause of contamination incident; length of exposure and level of contamination in microcuries (FCi), millirads, or millirems per hour. Airborne radioactivity is reported in FCi; saliva, skin, nasal, and wound contaminations are reported in alpha, beta-gamma, or neutron disintegrations per minute. Also included are medical response and decontamination activities, chelation, environmental impact, and the use of protective clothing and other equipment.
