These reports, along with detailed monthly summaries, are required by the Occupational Safety and Health Administration (OSHA) to document these events. Reports document three categories of accidents or injuries: 1) on the job and off the job; 2) reportable and nonreportable to OSHA; and 3) vehicular accidents. Data provides the name and job title of the injured employee; date, cause, and location of injury; description of treatment; and work restrictions imposed on the employee.
