Information indicates the facility, person calibrating the monitoring instrument, date of calibration, instrument number, background radiation in counts per 10 minutes (c/10 min) and counts per minute (c/m), radioactive source, source strength and source count, calibration factor, and date last calibrated and remarks. Some documents also provide employee name, social security number, work location, cause and location of wound, radiation survey log reference, actual and suspected contaminating isotope, date of incident, wound monitor number, time surveyed, results with background counts in c/10 min and c/m, and wound counts in disintegrations per minute.
