It discusses training and safety programs, waste handling systems, maintenance programs, and responses to earlier reviews. Of particular interest is a section reporting the melting of a fuel element in the ORR on July 1, 1963, which released an estimated 150 millicuries of iodine through the 250 foot stack. The report also includes recommendations for improving safety and preventing similar incidents.
