Originating Office: LANL, Occupational Medicine Group, Epidemiology Finding Aids: Shelf list Disposition Authority: Unscheduled Series Description: Records in Cabinet 7, Shelf 3, include the following: coding instructions for melanoma data; study correspondence, dated 1977-1978; lists of former Los Alamos employees diagnosed with melanoma, dated 1977-1978, which provide employee names, sex, dates employees were entered in the cancer accession registry, ages, cancer descriptions, current vital status, race, birthdates, Z numbers, diagnosis dates, and death dates; lists of employees with external exposure to beta, gamma, and neutron radiation and to tritium; lists of employees and employers (Sandia, Zia Company, Los Alamos Constructors, Inc.); data on Los Alamos residents diagnosed with melanoma, dated 1967-1975, which include residents' names, birthdates, diagnosis and hospital admission dates, hospital numbers, sex, ages, and death dates; summaries of the melanoma project, dated 1977-1978; and coding sheets for cases excluded from the study, which provide employee names, Z numbers, job titles, birthplaces, race, social security numbers, birthdates, sex, educational backgrounds, hire and termination dates, and marital status. Records in Cabinet 7, Shelf 5, include the following: a copy of a publication titled Analysis of Malignant Melanoma Incidence Among Employees at the Los Alamos National Laboratory from 1964 to 1978; lists of cancer incidence rates among various groups of Los Alamos employees; age-adjusted incidence rate data; "H-2" diagnosis reports, which list employee names, Z numbers, group numbers, diagnoses, ages, sex, race and employers (Zia Company or Los Alamos); a report, dated October 1982, which provides employee names, social security numbers, sex, birthdates, current vital status, and causes of death; data analyses, dated January 1981, listing cancer incidences in the state of New Mexico from 1969-1973; a computer-generated report, titled LAWM.MIG, which provides study numbers, employee names, social security numbers, sex, race, and job titles; notes concerning individuals deleted from electronic files because they were not Los Alamos National Laboratory employees; and an undated computer-generated report, titled INCV4T8.ALL, which includes employee names, social security numbers, birthdates, hire and termination dates, identificiation numbers, and exposure levels. Also on Shelf 5 is a New Mexico Tumor Registry (NMTR) Record Linkage Report, dated October 1982, which indicates employee names, addresses, social security and Z numbers, hire and termination dates, soundex numbers, sex, ethnic backgrounds, and birthdates. For those employees diagnosed with melanoma, the report includes NMTR account numbers, International Classification of Diseases (Oncology) numbers, cancer sites, current employee vital status, and, when applicable, causes of death.
