Originating Office: LANL, Occupational Medicine Group, Epidemiology Finding Aids: Shelf list Disposition Authority: Unscheduled Series Description: This series consists of Albuquerque Operations Office and Zia Company employee lists which were submitted to the Social Security Administration (SSA) to ascertain the vital status of former employees. The lists provide employee names and social security numbers. Also included are computer-generated reports and photocopied punchcards with information supplied by the SSA. One computer-generated report, titled Death--State Where Claim Was Filed, contains decedents' names, social security numbers, and death dates; cities and states where social security claims were filed; and facilities where deceased employees worked (indicated in numeric codes). A second computer-generated report, titled Death--State of Last Residence, contains decedents' names, social security numbers, death dates, facility codes, states and zip codes of last residence, and zip codes of respective funeral directors.
