Series Description: This record series consists of two primary types of records: employee record cards and health physics files of Dow Chemical and Rockwell International employees at Rocky Flats. The employee record cards identify the employer of each worker and document basic personnel information and employment histories. The record cards indicate employee names, current and previous addresses, previous employers, names and addresses of emergency contacts, hire dates, occupations, social security numbers, educational backgrounds, and military service. The cards also document all changes in salary and occupational titles and provide the reasons for those changes. They also include department names and organizational code numbers. Filed with the record cards are employee birth and death certificates.

The health physics files contain Radiation Monitoring Reports of Incidents, Accidents or Possible Inhalations, which provide incident locations and times, contamination levels, and descriptions of the contamination; Radioactive Skin Contamination Reports, which include diagrams indicating the extent and areas of skin contamination; Radiation Dosimetry Wound Count Reports, which include descriptions of wounds, gross and net count measurements, background readings, and medical actions taken; Body Counter Information forms, which provide employee names, identification numbers, body count dates and times, reasons for count, results in net counts per minute, and projected counts per minute for various body sites; Wound Counter Information forms, which include employee names, identification numbers, count dates and times, wound and source descriptions, and initial and final wound counts; and urinalysis reports, which indicate employee names and identification numbers, sample dates, and urinalysis results. Also included are annual badge reports, which provide employee names; identification and social security numbers; birth and hire dates; quarterly exposure levels for skin, hand, and penetrating radiation; and yearly and lifetime exposure totals.
