Series Description: The Health Event Audit Records consist of forms titled Health Event Audit which were used as cover sheets to photocopies of Report of Work Absences. The audit forms appear to abstract data from the work absence reports. This data was collected to monitor personal illness leave among exempt (salaried) and nonexempt (wage-earning) Hanford employees. The audit forms provide Health Surveillance System identification numbers, birthdates, sex, and employees' dates of absence. Information pertaining to disability, death, and malignancy diagnoses were completed when applicable. A narrative statement of the medical diagnosis, which often includes a description of the treatment, appears on the form. The work absence reports list employee names, social security numbers, job titles, supervisors, areas, and building locations.
